It is very important that everyone involved in an accident have a thorough examination by a physician or chiropractor. We work with your referring physician to assure continuity in your treatment. Early diagnosis and proper treatment by medical professionals assure you a steady recovery and gives you the best chance to avoid possible future physical problems.

Auto insurance information
A physician or chiropractor referral for massage therapy
Claims manager name, phone and fax number
Date of injury
Auto / PIP Insurance: We bill your Auto Insurance Company directly. There is usually no out of pocket expense associated with your claim.

WORK INJURY MASSAGE THERAPY

If you have been injured at work, we know how important it is for you to resolve the pain and discomfort you may have sustained in a timely manner. We work with your referring physician or chiropractor to coordinate a healing schedule that provides effective relief for many work related injuries. Our therapeutic treatment schedules have been successful in returning our clients to their normal and productive lifestyles in work and everyday living.

The following is a list of items we require to start treatment:

An open Labor and Industry (L&I) Claim

L&I insurance information
A physician or chiropractors referral for massage therapy
Claims managers name and claim number
Date of injury
L&I / Work Insurance: We bill your L&I / Work Injury Company directly. There is usually no out of pocket expense associated with your claim. 

**All appointments canceled with less than 48 hours notice will be charged $60.

Arlington Massage Therapy and Wellness has been operating in the Arlington / Marysville area for more than 18 years. When performing medical massage we keep in touch with your doctor. Our staff understands the different stages and phases of an injury and how it relates to your treatment. We keep your physician or chiropractor informed of your progress.

HOW MEDICAL MASSAGE CAN HELP

The soft tissues of the body include muscles, tendons, ligaments, fascia, joint capsules and nerves. In most cases, these tissues are the primary culprit that cause pain and dysfunction and can be easily and effectively treated with a comprehensive plan utilizing Medical Massage. Often, your pain can be eliminated permanently with a few visits by releasing chronically tight (hypertonic) tissues and restoring proper muscular health and balance to your body. In other cases, medical massage can reduce your pain, perhaps enough to allow you to avoid surgery or other invasive procedures. Medical Insurance:

Pricing varies dependent to each insurance company we contract with. If you are using your medical insurance coverage, we ask that all of our patients contact their own individual insurance company to find out their benefits. There will be a customer service line on the back of your insurance card.

Ask the following questions to your insurance:

1. Do I have in network Massage Therapy coverage
2. What is my co pay cost?
   What is my coinsurance cost?
   Does deductible apply? If yes, then ask how much to satisfy deductible for in network.
3. Do I have out of network massage coverage?
   What is my copay cost?
   What is the coinsurance cost?
   Does a deductible apply?
   How much to satisfy for out of network, if you have out of network coverage.
4. Is a Medical doctor or chiropractor referral required.
5. How many massage sessions allowed per. calendar year?

Claims may need to be pre-authorized and accompanied by a physicians referral. All medical insurance is billed electronically and usually has a turnaround of approximately two weeks. FYI – Confirm with your insurance that you have coverage with a massage therapist for massage services, Vs. a physical therapist or chiropractor. This is important.

Our treatment massage is a goal detailed massage treating  a specific injury or condition, which differs from our relaxation sessions.
