If you are using your network medical insurance benefits, your massage therapy sessions must be provided by a contracted massage provider with your particular plan. A massage therapy referral from an MD or DC will be required to bill your insurance plan.

When using your medical insurance coverage, we ask that our patients contact their insurance company to find out their individual benefits period. Check your customer service line on the back of your insurance card for benefits. We do not guarantee coverage for your insurance plan.

Medical Insurance:

Pricing varies dependent to each insurance company we contract with. If you are using your medical insurance coverage, we ask that all of our patients contact their own individual insurance company to find out their benefits. There will be a customer service line on the back of your insurance card.

Ask the following questions to your insurance:

1. Do I have in network Massage Therapy coverage
2. What is my co pay cost?
   What is my coinsurance cost?
   Does deductible apply? If yes, then ask how much to satisfy deductible for in network.
3. Do I have out of network massage coverage?
   What is my copay cost?
   What is the coinsurance cost?
   Does a deductible apply?
   How much to satisfy for out of network, if you have out of network coverage.
4. Is a Medical doctor or chiropractor referral required.
5. How many massage sessions allowed per. calendar year?

Claims may need to be pre-authorized and accompanied by a physicians referral. All medical insurance is billed electronically and usually has a turnaround of approximately two weeks. FYI – Confirm with your insurance that you have coverage with a massage therapist for massage services, Vs. a physical therapist or chiropractor. This is important.
